REQUEST FOR QUOTATION TO SUPPLY PERSONAL PROTECTIVE EQUIPMENT (PPE) TO
KNCV TUBERCULOSIS FOUNDATION, NIGERIA 

REFERENCE NO. – KNCVN-PPE-P/20/20-10-03

 General Information:          

KNCV is inviting vendors to provide quotations for the supply of PPEs.

Vendors are required to submit quotation indicating cost, payment, and delivery terms of the following items in the specified quantities

4,200 (Four thousand Two Hundred) units of KN95 Particulate Respirator

200 (Two Hundred) boxes of Disposable Latex free size gloves (box of 100 pieces)

1800 (One Thousand, Eight Hundred) bottles of Hand sanitizers (500ml bottle) with WHO approved mix of alcohol (ethanol), glycerol and hydrogen peroxide and Water

(Submit on both the attached KNCV RFQ and on your company letterheaded document) 

Fill and submit KNCV Vendor information form (form attached)

All bids must be written in English and signed by an authorized person

Bids submitted after the closing date/time will not be considered

Quotations submitted after the deadline for submission will neither be opened nor considered

A minimum of 5% WHT will be deducted per invoice as required by FIRS

PLEASE NOTE THAT VENDORS CAN SUBMIT BIDS FOR ONE, TWO, OR ALL ITEMS

Eligibility Requirements:

To be eligible to participate in this process, vendors must provide proof of all the following documents:

Company’s registration documents/certificate of incorporation with Corporate Affairs Commission (CAC)

Tax Clearance Certificate (For the Last 1 year)

Proof of provision of similar service to 3 different organizations.

(For first time bidders only – as Submissions in default of ANY requirement listed above will be excluded from the procedure).

Evaluation Criteria

The criteria for evaluation shall include cost, payment and, delivery terms.

Vendors are to submit requested documentation BY E-MAIL (only) – to procurement@kncvnigeria.org

“Name of your Organization” for PPE Supply – KNCVN-PPE-P/20/20-10-03

Selection process

The selected vendor will be contacted via e-mail, immediately after the selection process is concluded.

N/B

– Delivery Location – Independence Avenue, Central Business District, Abuja, Nigeria

– KNCV shall only accept from the selected vendor – “company banking details” not individual/private banking details

– KNCV shall not pay VAT (exemption certificate may be provided to selected vendor on request)
