PROCUREMENT AND INSTALLATION OF THE SECOND TRANCHE OF MEDICAL EQUIPMENT FOR ALL SECONDARY AND TERTIARY HOSPITALS IN THE STATE

(1.0) BACKGROUND
As a strategic intervention to improvement in the quality of services provided by the State’s secondary Health facilities an approval was given in 2017 for procurement of the first tranche of medical equipment. However, due to fund constraints, the Ministry was unable to provide all the equipment needed, hence the need to have a second tranche of equipment for procurement
(2.0) This National Competitive Bidding (NCB) process as specified by the State Procurement Guidelines ts open to all bidders from eligible countries as defined in the Guidelines — www.lagosppa.gov.ng.
(3.0) SCOPE OF WORK
The Lagos State Ministry of Health now invites sealed bids from eligible contractors for the contract below as detailed in the bidding document

(4.0) GENERAL QUALIFICATION REQUIREMENTS FROM INTERESTED PARTIES
(i) Evidence of Registration with Corporate Affairs Commission (CAC) with inclusion of Certificate of Incorporation and Articles of Association.
(ii) Company profile and Organizational structure with full name of Organization and contact person, postal address, telephone numbers, and e-mail addresses.
(iii) Evidence of company tax clearance for the last 3 years (2017.2018&2019)
(iv) Audited financial statements of the company for the years 2017, 2018 &2019.
(v) Evidence of 3 Years Development Levy for two Directors
(vi) Copies of VAT Registration with TIN number and evidences of VAT remittances for the last 3years
(vii) Evidence of available financing and/or access to credit line.
(viii)Verifiable evidence of 2 similar experiences (within a minimum of 3 Years) with Government, Private or Non-Governmental Organizations (These should include evidence of satisfactory completion or progress for each experience indicated)
(ix) A sworn Affidavit that all documents submitted is genuine and verifiable.
(x) Evidence of registration with Lagos Stale Government (PPA) in the appropriate class
(5.0) NOTES
(a) Additional details are provided in the Bidding Documents
(b) All claims must be adequately substantiated with documents and must be verifiable.
(c) The Ministry of Health shall deal directly with only the authorized officers of interested companies and not their agents
(d) This bidding exercise shall not be construed as a commitment on the part of the Ministry of Health to award the contracts to anyone or even award any contracts at all.
(e) The Ministry of Health will not be responsible for any cost or expenses incurred by any interested party(ies) in connection with site inspections or responses to this invitation.
(6.0) COLLECTION OF BID DOCUMENTS
(6.1) Only Vendors who are already registered with the Lagos State Public Procurement Agency (LSPPA) and are in its vendor data base shall be able to access and download the Bid Document
(6.2) Interested bidders who are yet to register, should quickly visit the PPA website www.lagosppa.gov.ng) for Contractors Registration and Guide.

(7.0) SUBMISSION OF BID DOCUMENTS
(7.1) Bidders shall upload their bids virtually through the same electronic medium by which the bid was downloaded
(7.2) Please note, interested bidders are to ensure their business registration with PPA are in the right category and classification
(7.3) Uploading of bids should be completed on or before 12.00 noon (mid-day) Monday 10 August 2020
(8.0) BID OPENING
(8.1) The electronic Bids will be opened and projected in the presence of the bidder or bidder’s representative who chose to come to the conference room of the Ministry of Health at 12.00 noon on Monday 10 August. 2020
(8.2) ADDITIONAL INFORMATION
All request for clarifications should be directed to the Head of Procurement via email at orolatunji@lagosstate.gov,ng on or before 12:00 noon on Friday 7 August. 2020.
